TERMS AND CONDITIONS

In order to register for an event hosted by Simcoe and District Minor Hockey Association, this form must be completed in full.
Completion of this form does not guarantee the submitting team a spot in the event. Registration is on a first come, first served basis and isn't considered complete until payment has been received.
Registration forms, rosters and travel permits will be accepted by online/email submission only.
Payment must be received within three weeks following the submission or the registration form in order to guarantee the submitting team a spot in the event. Payments may be made by cheque or Etransfer to [email protected]. Please include team name and tournament name.
Event fees are non-refundable (unless the spot can be filled).
